Planning a change? Tell the Director of Safety here. Some changes need a Management of Change (MOC) review before they take effect, so the risks are found and handled first. You do not have to know whether yours does: if it might affect safety, send it. No login needed.

4Who it affects and what you see
Departments affected

Names or groups. For example: all Praetor 600 crews, hangar staff, charter sales.

A hazard is a condition or an object with the potential to cause or contribute to an aircraft incident or accident (ICAO Annex 19). List any you can see. Leave blank if none come to mind.

Training, briefings, documents, vendor checks, or anything else already in place.

What happens next. Submitting does not approve or stop the change. The Director of Safety reviews it and decides whether a Management of Change record is opened (SMS Manual §5.8).

Notices go to the Director of Safety, who may contact you with questions.
